Immunization Requirements for All Students in NWABSD

Prior to first entry in an Alaska public school district or nonpublic school offering pre-elementary education through the 12th grade or any combination of these grades, a child shall be immunized against diphtheria, tetanus, polio, pertussis, Hepatitis A & B, mumps, measles, rubella and varicella (chickenpox) 2 doses for students in grades K through 6th. Any student who does not show evidence of required immunization or who does not present a letter or affidavit from the parent/guardian or physician, physician's assistant, or advanced nurse practitioner stating reasons for exemption based on medical reasons or personal beliefs, shall be excluded from school until such time as the immunization is obtained or affidavit of exemption has been filed with the school.

It is the parents' responsibility to make certain these requirements are completed. The school will remind you; but the policy is "no shots, no school".

Registration

Please visit your school secretary. Students entering school for the first time or students transferring from another district will need the following items for registration:

· Current record of immunizations

· Birth certificate

· Record of physical examination (to participate in sports)

Students must meet the following grade/age requirements by September 1: Pre-K-4 years of age; Kindergarten-5 years of age; 1st grade-6 years of age.
